- Photocopy of the COC and endorsement to it
- Photocopy of the GMDSS certificate and endorsement to it (if applicable)
- Photocopies of all certificates required for the current rank
- Photocopy of the seaman's book
- Photocopy of the international passport
- One photo for the application form
- Sea service reports (if available)
